When is a CARE assessment required?

• When an individual wants to be admitted to a Medicaid-certified nursing facility (NF) or long-term care unit (this includes individuals coming from their community home, an Assisted Living, Residential, or Boarding Care Home); and
 
• The individual does not have a valid proof of PASRR.
 
• Exceptions to this rule are referred to as provisional or categorized admissions.
 

 

 

 

  Note:  Individuals must be at least 16 years or older to be admitted to a nursing facility in Kansas.
